I currently am unable to view form submissions on my end. Any thoughts on why this is happening - we've cleared caches and rebooted multiple times.

From the image, it looks like you have a CSS conflict. Since the checkboxes are showing, I'd the CSS is rendering the page with foreground and background colors the same. User your browser's inspection tool to check your CSS. There's not other way to help you diagnose a problem in the admin area. You'll have to figure this one out yourself.
