My forms with conditional logic are not loading.  Is this a javascript issue? Previously working forms are no longer loading after a recent hosting change. Trying to work out the issue.

Check your browser's console log for errors.

Make sure you're using Formidable Version 6.15.1. There was a problem with 6.15 that broke a few things: https://formidableforms.com/changelog/formidable-forms-6-15-1/
