How does one disable entries in a dropdown lookup field previously submitted?

Example:

I have a field [23] in form [A] that looks up values from another field [12] in another form [B]. I want any value that has been selected and submitted on form [A] to either be disabled (greyed out) or not show up in the list when it is looked up by field [23].
