Do you have any custom HTML fields that may be missing a closing tag? That's the usual reason for HTML not generating correctly. I'd inspect the page's source code and if possible, run it through a syntax checker.
Hi Victor.
No, the html is fine. The form works perfectly for a few days, and then after a lot of users are submitting it, it seems to break the layout somehow in the admin of the form.
The only thing I am wondering is because of the amount of people using this form, and the fact that it is a large form with a lot of conditionals, one of the php settings on the server could perhaps cause an issue? It is the only things I can think could cause any problem of this kind.

Is the form excluded from all caching?
I don't have any cache plugin enabled on the website at all
What about server and browser caching? It doesn't have to be a WordPress plugin?
It doesn't appear to be a caching issue from what I can tell. The form works well for a few submissions, and then at a random point appears to move some of it's fields out of a section to below it. It's the strangest of malfunctions I have seen in a while.
I'm at a loss. Since this is the first time anyone has written about this issue, it seems to be isolated to your installation and requires a much deeper level of troubleshooting than we can provide as community volunteers.
