Are all of your Formidable plugins updated to their latest versions? The current versions of Formidable Forms and Pro are 5.2.05 and the User Registration add-on is 2.04.

If you don't have the latest versions, please update the plugins. If everything is on the latest version, then you have another system problem and I recommend you open a support ticket with the Strategy 11 team.
