Hi and thanks for any support in advance. I have a basic form with four questions and I am seeking to apply a conditional message based on the outcome. I am using radio buttons with a simple Yes or No option.

I have used separate values for the radio with the text being Yes or No and adding values to the Yes or No fields.  

The questions are broken into two categories.
Category 1 has three questions.
Category 2 has one question.

As stated above, the answers are supplied by radio button Yes or No responses.

I have created a hidden field that adds up the responses from the four questions which have values attached to the yes or no selections.

Based on the hidden field sum, the user will receive one of two form submission messages indicating either a pass or fail status In an ideal world the form would also include a redirect to one of two corresponding urls based on the same logic however for now, I have disabled/omitted this as I cant even get the conditional messages to work as intended.

The Question Scoring Values are as follows: Question 1: Value 1 for Yes and Value 0 for No
Question 2: Value 2 for Yes and Value 0 for No
Question 3: Value 4 for Yes and Value 0 for No
Question 4: Value 10 for Yes and Value 0 for No

I have created two form ‘Confirmation Form Actions’ for the intended messages and correlated Conditional Logic.

One for a pass and one for a fail.  
Sum Values equating to a Pass

In order to achieve a Pass, the user must select Yes for at least two of the three questions in Category 1, plus a Yes selection to Q4 in Category 2.

The Pass variables are detailed below:

1. Score ‘equals’ 13 (Q1,Q2 and Q4), or
2. Score ‘equals’ 15 (Q1,  Q3 and Q4), or
3. Score ‘equals’ 16 (Q2, Q3 and Q4), or
4. Score ‘equals’ 17 (Q1, Q2, Q3 and Q4)

ERRORS as follows:

1. I receive a pass and a fail message
2. I receive a pass and a fail message
3. I receive a pass and a fail message
4. I receive a pass and a fail message

A Fail would result in a sum ‘does not equal’ to any of the above sum values in the Hidden field.

The Fail variable are detailed below:

1. Score ‘does not equal’ 13 (Q1,Q2 and Q4), or
2. Score ‘does not equal’ 15 (Q1,  Q3 and Q4), or
3. Score ‘does not equal’ 16 (Q2, Q3 and Q4), or
4. Score ‘does not equal’ 17 (Q1, Q2, Q3 and Q4)

All the above work as intended presently. Hidden 1 field in form Advanced Default (Calculation) is as follows: [q1] + [q2] + [q3] + [q4] The Hidden 1 works as I can see the correct scores on submission. Problem:

1. I am receiving both messages simultaneously (Pass and Fail) which should be impossible.
2. Alternatively I receive multiple (duplicate same messages) with certain options, for example in (Pass 4) above. This is perhaps more trickly to resolve.

The messages displayed currently used rich formatting with links contained for alternative options for the user.

In conclusion, I’ve been working on this for days which I find amazing as the objective is so simple.

I’m clearly a newbie to Formidable having migrated from Caldera.
I would be eternally grateful for any support or guidance.
