Please refer to the screenshots below
I am hoping to accomplish something similar to what is shown. On selecting a certain radio button, a modal is triggered and presents a text box field.
That text is then issued to a separate long-text field below the series of m/c questions.
Is this possible in formidable? using bootstrap? is it difficult if you don't know anything about code? Thanks.

Instead of using a modal window, which requires a different form, use a field that opens conditionally,under or adjacent to the question. This won't require custom code where your solution may to link the forms together.
