Locations now only cascades one field. I did a clean install going all the back to 5.5.7 with lite and pro but could only find 2.03 for the locations plugin. Only those three plugins are installed. Cascading fields fail when a third field is used. I took off the watch for country and it worked for three fields temporarily for some reason. It now fails again.
Have you cleared all browser and server caches? Since the problem has only been reported by you and my tests show your site is working fine, it sounds like something is going wrong in your local environment.

State North Carolina but not a city in the email I got.
The front end seems to work fine, but the city field wasn’t added to the database? Did you examine the entry’s metadata with PHPMyAdmin? If you check the database, it may help isolate the issue.
That is way out of my pay grade, just like steaks were out of my paramedic pay grade! I am going to put the country field on view just for administrator and put the default to United States to see if that will work.
