Form already submiitted error message

Hi   What are the likely causes of "We're sorry. It looks like you've already submitted that. "  when the form has been submitted. It appears that despite this error message, the entry is still created but it is bad to display such an error message.   Please advise.   Thanks.   Henry

Make sure caching is turned off for the form.
