Lag or Errors in Entry Logging

Is it common for a form to lag or have error in entry logging? I'm currently using the forms for a fairly large (about 3000 person) tournament for submissions. The way the tournament schedule is set up, entries can bottle neck all at the same time as people race for the submission deadline. I would say it currently has about a 99% accuracy in what's recorded in the log as opposed to what people are expecting for their results. However there are about 1% discrepancies, which in this instance can be a dozen instances per week. People insisting that they submitted one thing, but the entry log shows another. People insisting they submitted something on time but the entry log shows it submitted after. For my own peace of mind, is the entry log usually 100% accurate for what was submitted in a form, or is it possible that the discrepancies players report are valid?

3,000 people in a tournament sounds like there’s a lot of I/O going on which means performance issues can be anything from not enough php workers to database problems. It could bandwidth. What kind of server do you have?

Its a 16gb 8cpu server. The cpu % and i/os usually are never an issue.
