Multiple ways to Lookup

I currently have a table database that consists of Item Number, Item Name, Price. On Front-end, I created a form where the the user enters the Item Number, and the Item name & Price and populated with simple Lookup field, and then enter a quantity enter a Quantity field.

How can I give users the option to Search for the Item name instead...just in case the Item Number is damaged/illegible?

I don't understand what you mean when you say "just in case the Item Number is damaged/illegible". When you create a search form, you can add any fields you want to it.

Sorry, I meant to say the the printed item number on the physical item.
