Hi there,
I've updated to the recent 6.13.3 patch and my forms titles and descriptions won't show atop my forms anymore even if both options are checked under the Settings page. See attached screenshots for context. Here are short descriptions of what's on each screenshot:

  1. (Settings) Title and Description are checked and enabled.
  2. (Preview - In theme) Title and description are showing correctly.
  3. (Actual page) Title and description aren't showing and are not in code.

Can anyone assist?

How are you displaying the form's shortcode on the live page? Make sure the shortcode options for displaying the form title and description are enabled. If you are using the block builder, there are toggles for this in the shortcode's control panel.
