Hi, I was failing to use the conditional logic options to accomplish exactly what I was trying to do. I also failed to asked the question fully. What I was needing to do for both of my clients was create one form with multiple confirmations (or email notifications) based on the sub-page the form is on. It is also worth noting that I am converting both these clients over to Formidable Forms from Gravity Forms.  What i was doing wrong was in the conditional logic... I needed to use the "Contains" option instead of the "equals" option to make the form forward (or email) the pdf correctly.
