This gets a bit deep, but you'll probably want to use a combination of https://www.fdmdigital.co.uk/formidable-form-timer/ and https://www.educba.com/jquery-read-only/?source=leftnav. You'll need to get a time up on the form, auto start it, then have a function for after a period of time make all fields read-only and/or redirect as you wish.
