I need to get price from another form by using lookup field as checkbox  (also using Filter by Lookup Fields for comparing fields) some has same price values.. I want duplicate entry to be allowed in lookup

I use two checkbox to filter (Parts name checkbox) & (price Checkbox)

if i check parts name the relevant price should come..

Due to duplicate removal its not showing.. so kindly help or provide code:)
