I just upgraded to the business pro plan, to utilize Likert fields.

The only challenge is that when I run and download the CVS Report for this form, none of the Likert field selections are captured in the report attached.
Isn’t Formidable Forms’ data capture mechanism capturing Likert data like all other fields?

On the dashboard, you go to Formidable > Entries > Select the form preferred > Click the Download CSV button. See screenshot attached.
That function downloads all data for all entries. I just tested it in my development environment and it works fine.

Thank you for taking the time to help.
I updated the field options for Likert to include “Separate Column Values”.
The fields are now showing on the CSV report, however, they are all duplicated (see attached). For example:
Cell M1 says, Services Supported - Truckload (label)
Cell N1 says, Services Supported – Truckload
This is occurring in the Likert fields only.
Is there a setting I need to adjust?
