I want to have someone come to a page, enter their name and some other data, watch an educational product video, then take a quiz and if they pass send a certificate. There will be 1000 or so people and somehow I'd like the product manager to be able to view who's taken the quiz and their scores. A report of some sort for the product manager.
What steps would I need to take to get this going?

You need Formidable Pro to build the form, Visual Views for the report, the quiz maker and forms to pdf add-ons. Formidable handles requirements like this easily if you understand the product.

Do I create a form with the name and email, etc. then upon submit have it go to the quiz form? Or is it all done in one form?
It can all be done in one form, even user registration if creating WordPress accounts is a business goal.
