I accidentally deleted some fields on my form, can I restore them?

I think the data still exists in the database. Is there a way to add the fields back into the form? There is only a "Add [new] Field" option. I would need to retrieve the deleted fields and retain the id's and data associated with those fields. Is this possible?

Once you delete fields, the data is gone. It no longer exists in the database. There's no way to add them back. Your only option is to restore from a backup you created before you deleted the fields.
